Background & objectives: Respiratory diseases are the most prevalent disorders caused by exposure to chemical pollutants in the workplace. Flour is one of the allergen dusts in the workplace that may cause respiratory damages. This study aimed to assess flour dust concentrations and its relation with lung function parameters among Zahedan flour mill workers.

Methods: In this retrospective cohort study, 30 samples were collected from breathing zone of workers in 6 flour mill industries by a personal air sampling pump and a cyclone using NIOSH method 0600. Respirable dust concentrations were measured gravimetrically. Lung functions of flour mill workers were compared to those of non-exposed employees and statistical analyses were done using SPSS 18 software.

Results: Mean age of case and control groups were 37.93±7.5 and 40.53±11.45 years, respectively. Significant difference was observed in PEF values between case and control groups.  Amongst all the lung functions in 3 consecutive years (2012-2015), significant relationship was only observed between FEV1 and flour dust concentrations (p<0.05).

Conclusion: Present study showed significant relationship between flour dust concentration and pulmonary function changes. Excluding silos and packing sections, dust concentrations in all other sections of the factories were within the standard range. However, slightly variation in flour dust concentration can affect workers pulmonary functions in short term (i.e.1-2 year).
